Privacy & personal information
Recovera collects health information to provide nursing care at home. This notice explains what we collect, why, how long we keep it, and the choices you have.
Consent wording version v1-2026-09
Who is responsible for your information
Recovera is responsible for the personal information in our care. Our Privacy Officer answers questions, handles requests to see or correct your information, and manages complaints.
Privacy Officer: [name and title] · [email] · (604) 404-7640
What we collect
- Your name, phone number, and email address.
- Who you are to the patient (yourself, a family member, a surgeon's office).
- The type of surgery, the surgery or discharge date, and anything you tell us about the recovery.
- Care notes our nurses create during visits, once care begins.
We only ask for what we need to plan and deliver safe care. You can leave optional fields blank.
Why we collect it
- To contact you about your care request.
- To assess whether home nursing is appropriate and to plan the first visit.
- To coordinate with your surgeon's office or hospital, with your consent.
- To keep the clinical records a licensed nursing practice is required to keep.
We do not sell your information, and we do not use it for advertising.
Your consent
We collect, use, and share your information only with your consent, except where BC law allows or requires otherwise — for example in a medical emergency, or where a regulator or court requires disclosure.
You may withdraw consent at any time by contacting our Privacy Officer. If you withdraw consent we may no longer be able to provide care. Some records must still be retained for the periods described below.
Who can see it
Access is limited to Recovera staff who need the information to do their job. Every staff member signs in with their own account, and every time a record is opened or changed it is recorded in an internal access log. New accounts have no access at all until an administrator grants it.
We share information outside Recovera only with your consent — typically with your surgeon, family doctor, or a family member you name — or where the law requires it.
Where it is stored
Information is stored in encrypted databases hosted in [region — confirm and state the hosting region]. If any of it is stored or processed outside Canada, the laws of that country may allow its government or courts to require access. By submitting the form you consent to storage and processing in that location. Contact our Privacy Officer if you would like details or prefer another arrangement.
How we protect it
- Encrypted in transit and at rest.
- Individual staff accounts with strong passwords; no shared logins.
- Access limited by role, and reviewed regularly.
- An access log showing who viewed or changed each record.
- A documented process for responding to a privacy breach, including notifying affected people.
How long we keep it
Care requests that do not become active care are kept for [retention period, e.g. 1 year] and then securely deleted. Clinical records are kept for the period required for nursing records in British Columbia — confirm the applicable period with your regulator and legal advisor — and then securely destroyed.
Seeing or correcting your information
You have the right to ask what personal information we hold about you, how we have used it, and who we have shared it with. You can also ask us to correct anything that is wrong. Contact our Privacy Officer and we will respond within 30 business days, as PIPA requires.
Complaints
If you are not satisfied with how we handled your information, please contact our Privacy Officer first. You may also complain to the Office of the Information and Privacy Commissioner for British Columbia.
